International generic name Potassium chloride
Drug form and composition Solution for injection. One ampoule of 10 ml contains 1.5 g Potassium chloride (0.15 g /ml).
Indications Cardiac rhythm disorders due to intoxication with digitalis glycosides, and hypokalemia of different causes - thiazide diuretics overdosage, treatment with corticosteroids and purgative agents.
Dosage and administration The solution is applies as an intravenous infusion; 1.5-3 g Kalium chloratum (1-2 ampoules) is diluted in 500 ml 5% glucose solution. In patients with severe digitalis glycoside intoxication 3 g Kalium chloratum, diluted in 500 ml 5% Glucose solution, is applied intravenously (intravenous drop infusion within 1 hour); the dose should not exceed 1.5 l /24 hours solution. Kalium chloratum should not be used in dose higher than 1.5 g per hour.
Contraindications Hyperkalemia of different causes; Addison’s disease; no controlled diabetes; renal failure; ulcer disease; acute dehydration; oliguria; anuria; hyperchloremia.
Special warnings and precautions Intravenous application of the preparation should be made very slowly, under close control of the blood pressure. The preparation should be used with great attention in pregnant and nursing women.
Drug interactions Concomitant administration of potassium iodide and iodine containing X-ray-contrast media reduces the preparation effectiveness. Combination with ACE-inhibitors, anticholinergic drugs, β-adrenergic blockers, heparin, and potassium-spearing diuretics is not advisable.
Adverse reactions Kalium chloratum may cause following adverse reactions: heart rhythm disorders; paresthesias; nausea; vomiting; diarrhea; epigastral heaviness; gastrointestinal tract ulcerations.
Pharmacological mechanisms Potassium ions play an important role in the electrolyte balance of the cells and have an important significance in nervous impulse conduction, in muscular contraction, and in heart automaticity maintaining. The sodium/potassium ions ratio in the cell is of main significance in the proper functioning of the myocardium, vegetative tone maintaining, and protein and glycogen exchange. Potassium ions participate in the maintaining and excitation of the sympathetic nervous system. They possess a moderate diuretic effect also.
