Drug form and composition Syrup. 5 ml Syrup contains 12 mg Diphenhydramine hydrochloride; 6 mg Codeine Phosphate Hemihydrate; 100 mg Ammonium chloride.
Indications The preparation is used in inflammatory diseases of the upper respiratory tract; acute and chronic bronchitis; tracheobronchitis; pertussis. It depresses the irritative and spastic cough.
Dosage and administration The syrup is administered in following dosage: children, 6-8 years - 2.5-5 ml 3-4 times daily; children, 9-14 years - 5 ml 3-4 times daily. In adults and children older than 14 years - 10 ml 3-4 times daily.
Contraindications Hypersensitivity to any of the preparation ingredients; acute asthmatic attacks; narrow-angle glaucoma; prostatic gland hypertrophy; ureteral stenosis; pregnancy and lactation; children younger than 6 years.
Special warnings and precautions In infants the preparation should be dosed carefully, as it may cause excitation and convulsions. Usage of the preparation in elderly persons with severe constipation is not recommended. Dimex should be used carefully in patients with bronchial asthma because it impedes secretion elimination. During the treatment alcohol consumption is undesirable. Dimex-Co should not be used in driving and machinery operating persons, due to possibility to provoke somnolence and coordination disorders.
Drug interactions Hypnotic agents and alcohol potentiate the sedative effect of the preparation. It should not be combined with MAO-inhibitors. Concomitant treatment with tricycle antidepressants may cause an additive synergism concerning the anti-cholinergic effect of the diphenhydramine with resultant urine retention, intestinal atonia, enhancement of the ophthalmic tone.
Adverse reactions Somnolence; nausea; headache; attention disorders; weakness; visual disorders; blood count changes; insomnia; anxiety; tremor; drug-dependence; constipation.
Pharmacological mechanisms The three main constituents of the medicine determine its antitussive, secretolytic, broncholytic and antiseptic action. Diphenhydramine (H1-blocker), influence the cough reflex, depresses the allergy reactions, reduces the bronchial secretion, and causes sedative and antiemetic effects. Ammonium chloride liquefies the bronchial secretion and facilitates its elimination. It exerts some stimulating action on the respiratory center, which improves the expectoration-stimulating effect. Codeine phosphate, being an opium alkaloid, improves the antitussive effect of the preparation, depressing directly and selectively the cough-center in the medulla oblongata. It possesses some analgesic and sedative effects also.
