COMPOSITION:
Aminophylline 480 mg/2 ml; 240 mg/10 ml in one ampoule

ACTION:
Novphylline is a compound of theophylline (78 %) with ethylendiamine (22 %), with additive action of both components, connected with inhibition of enzyme phosphodiesterase. It possesses a spasmolytic and vasodilating action, and decreases the resistance of the vessels. It dilates the coronary vessels and stimulates cardiac contractions, increases the stroke and minute volume, while raising the need of oxygen in the cardiac muscle. It removes the spasms of the smooth muscles of the blood vessels, bronchi and intestines. It enhances the diuresis by increasing the glomerular filtration. It also improves cerebral circulation, reducing the intracranial pressure and the cerebral edema in the case of insult. Novphylline has a certain antihistamine and desensitizing action.

PHARMACOKINETICS:
Novphylline metabolizes in hepatocytes, only 10 % of it being excreted unchanged by the kidneys. In children it metabolizes more rapidly. Its plasma half-life is from 6 to 8 h. the therapeutic plasma concentration is from 10 to 15 mkg/ml (maximum concentration 20 mkg/ml).

INDICATIONS:
Cardiac asthma, especially when the seizures are accompanied by bronchospasm or the appearance of pulmonary edema. Ischemic cerebral insult, vascular cerebral crises, transitory circulation disturbances.
Bronchial asthma, especially with symptoms of congestive insufficiency in the lesser circulation. It is applied also in leucopenia and agranulocytosis.

CONTRAINDICATION:
Novpjyllin should not be applied in cases of cardiac insufficiency, especially if it is connected with myocardial infarction, in coronary sclerosis and cardiac rhtythmic disturbances.

PRECAUTIONS:
A sharp AP rise may occur after intravenous application.

SIDE EFFECTS:
Oral administration: dyseptic symptoms (nausea, vomiting, diarrhea); administration in the evening-insomnia. Rapid intravenous application - dizziness, headache, palpitation, nausea, convulsions, sharp drop of AP. At overdoses - rare toxic symptoms connected with overexcitation of CNS (restlessness, dizziness, convulsions).

DRUG INTERACTIONS:
Contradictory evidence for increased effect of corticosteriod preparations. The preparation is not applied in combination with oleandomycin.

DOSAGE AND ADMINISTRATION:
Intramuscularly: 240-480 mg
Intravenously: 120-240 mg Novphylline diluted in 10-20 ml of 20 % or 40 % glucose solution, injected very slowly (for 4 to 6 min).

MEDICINAL FORM AND PACKAGE:
Ampoules of 480 mg/2 ml and 240 mg/10 ml in packs of 10 and 50.

STORAGE:
At moderate temperature (15-30° C).

EXPIRY:
2 (two) years.
