- COMPOSITION:
Piroxicam
- 10 and 20 mg in one capsule and
20 mg in one suppository
ACTION:
Piroxicam is a non-steroid antiphlogistic agent from the group of
oxicams. It has a strong antiphlogistic, analgesic and antipyretic
effect. Its mechanism of action is primarily due to the inhibition
of prostaglandins synthesis. Similar to other non-steroid
antiphlogistic agents, piroxicam inhibits the second phase of
platelet aggregation.
PHARMACOKINETICS:
It has a very good gastrointestinal absorption. It binds with plasma
proteins in about 98 %. Its plasma half-life is about 40 h, making
it suitable for administration in single doses. About 95 % of the
drug is metabolized and excreted mainly with the urine, and only an
insignificant part with the feces.
INDICATIONS:
Inflammatory articular diseases - rheumatoid arthritis, ankylosing
spondylarthritis, etc.; highly activated and painful degenerative
articular diseases - arthrosis, spondylosis; acute stage of
rheumatism of soft tissues - bursitis, synovitis, tendovaginitis;
acute painful posttraumatic and postoperative states; acute gout
attack.
CONTRAINDICATIONS:
Gastric and duodnal ulcer, susceptibility to bleeding,
hypersensitivity to aspirin or other non-steroid antiphlogistic
agents (particularly if they have induced asthmatic paroxysms,
allergic rhinitis or urticaria), pregnancy and breast-feeding. It is
not administered to children below the age of 8 years.
SIDE EFFECTS:
Gastrointestinal disturbances - nausea, sense of heaviness,
diarrhea, dyspersia, peptic ulcers, hemorrhages from the
gastrointestinal tract. Those disturbances are usually
dose-dependent.
As regard the CNS - sleep disorders, headache, dizziness may be
observed.
The most frequent skin reaction are rashes, itching,
photosensitivity. Cardiovascular and renal disturbances, suppression
of bone marrow function (granulocytopenia, anemia) may occur very
rarely. |
DRUG INTERACTIONS:
Systematic medical control is necessary in simultaneous
administration with pharmaceuticals that bind with plasma proteins
to a high percentage. Aspirin and other non-steroid antiphlogistic
agents reduce the effect of piroxicam (due to competition for serum
proteins). Piroxicam diminish the effect of diuretics and other
hypotensive agents, while enhancing the effect of lithium drugs.
PRECAUTIONS:
Piroxicam can weaken attention and reactions, which should be taken
into consideration when the drug is prescribed to drivers and
machine operators. That action of piroxicam is enhanced by
simultaneous intake with alcohol. It should be administered
cautiously to petients on maintenance anticoagulant therapy, with
regular control of paraclinical indices.
DOSGAE AND ADMINISTRATION:
Capsules: a single dose of 20 mg is administered daily to adults. In
more severe and painful conditions - a single dose of 40 mg. After
the abatement of the acute complaints - 20 mg daily.
Suppositories: one suppository daily is applied to adults, and the
dose can be repeated in very acute inflammatory processes with
concomitant pain syndrome. To children from 8 to 16 years - 1
suppository daily. Maximum course of treatment - from 5 to 8 days.
MEDICINAL FORM AND PACKAGE:
Packages with 20 capsules of 10 and 20 mg.
Packages with 5 suppositories of 20 mg.
STORAGE:
In a place protected from light, at a temperature from 15 to 30° C
for capsules and 5-15° C for suppositories.
EXPIRY:
3 (three) years.
