- COMPOSITIPON:
Ergocalciferol 300 000 IU/1 ml; 600 000 IU/1 ml in one ampoule
and 400 000 IU/10 ml oil solution.
ACTION:
Ergocalciferol (D2) has the highest activity from the group of
Vitamin D. it regulates: phosphorus-calcium metabolism and the
processes of ossification; absorption of phosphorus and amino acids
in renal tubules.
Vitamin D is absorbed in the jejunum wall and as calciferol crosses
into the blood plasma and lymph. Its absorption depends on the
functional condition of the pancreas and is strongly reduced in the
presence of steatorrhea. The half-elimination time of Vitamin D is
26 h. Calciferol is metabolized in the liver to the polar compound
1.25-dihydroxy-chole-calciferol which possesses more strongly
manifested pharmacological activity than the basic substance and its
half-elimination time is between 7 and 14 days.
INDICATIONS:
It is used prophylactically and for the treatment of rickets,
spasmophilia and osteomalacia, for acceleration of the coalescence
of fractured bones, protection from dental caries, in osteoporosis,
hypocalcemia, hypoparathyroidism, scrofula, exudative diathesis.
Vitamin D is also administered for the treatment of lupus, pleurisy
and tuberculosis of the larynx, kidneys, etc.
CONTRAINDICATIONS:
Elevated level of calcium in the plasma, hypervitaminosis A and
disordered renal function.
SIDE EFFECTS:
Vitamin D overdosage causes acute intoxication as in intoxication
with parathyroid hormone: loss of appetite, diarrhea, polyuria;
elevated calcium concentration in the blood; calcium is deposited in
the renal tubules, blood vessels of the heart, bronchi and stomach;
degenerative phenomena in the liver are observed. The toxic doses
for adults amount to 100 000 IU daily, taken for months, in children
they are between 20 000 and 40 000 IU daily, also after a long-term
intake. |
DRUG INTERACTIONS:
Antacids, anticonvulsants, sucralfate and mineral oils reduce the
effect of Vitamin D2, as they decrease its resorption. Enhanced
toxicity of digitalis drugs is possible, because Vitamin D2 can lead
to hypercalcemia.
DOSAGE AND ADMINISTRATION:
The daily need of Vitamin D is 400 IU on the average. In light forms
of rickets the vitamin is administered in a dose of 5 000 IU daily,
whereas the severe forms require an increase of the dose to 10 000
IU daily.
The solution is administered for therapeutic purposes - 3-5 drops
daily in milk. Prophylactically - 1-2 drops daily for 20-30 days
with a pause of 10-14 days.
The ampoules are administered intramuscularly in high doses in
certain cases: e.g., 20 000 IU to breast-fed infants and 50 000 IU
to children, but then the calcium blood level should be checked
every week. In certain cases of rickets the so-called "shock
therapy" is carried out with Vitamin D2 - a single
intramuscular injection of 300 000-600 000 IU (active in the course
of 2 months) or 500 000-1 000 000 IU for 3-5 days, intramuscularly.
In skin and extrapulmonary tuberculosis, orchiepididymitis,
laryngitis, etc., lichen ruber planus - 1 ampoule = 600 000 IU (=
0.015), three times weekly during the first week, twice weekly
during the following 3 weeks, once weekly from the fifth week on to
the end of the 16th week, and when necessary - 1 ampoule every 2
weeks till the end of one year.
MEDICINAL FORM AND PACKAGE:
Packages with 10 and 50 ampoules of 1 ml.
Oil solution in vials of 10 ml.
STORAGE:
At moderate temperature (15-30° C), in a place protected from
direct sunlight.
EXPIRY:
1 (one) year.
